I understand the nature of this outing in which my child is participating. I also acknowledge that my child is expected to follow and abide by the rules and regulations for all Arc community outings. When riding in the Arc van, all passengers MUST remain seated and MUST wear seat belts. I understand that the Arc van is not wheelchair accessible.
I acknowledge that the Arc’s staff are not licensed or trained to provide medical treatment other than basic first aid and basic life support. I acknowledge that Arc staff are not licensed or trained to dispense medication. If my child has any severely impacting medical conditions such as a seizure disorder, I will accompany him or her on the trip.
I hereby release The Arc Susquehanna Valley from all liability for any injuries sustained by my son/daughter/child in my care, including specifically all claims, demands, negligence, actions, and judgments sustained by my son/daughter/person in my care, or myself.
